Supported Lifestyles Ltd. is a not-for-profit agency that provides services to adults with developmental disabilities. At Supported Lifestyles Ltd. there are a number of service areas that provide specialized services to our clients.

The Client Safety (CS) Coordinator will monitor and evaluate health and safety indicators brought forward by front line staff, supervisors, clinical team, guardians and directors and will work with teams to create a plan to increase client safety. This position will review and investigate all client injury as reported through critical incident reports. The CS Coordinator will be responsible to ensure all client injury investigations occur in a thorough and timely manner. Specific responsibilities include significant time spent directly observing, monitoring, and testing all aspects of client risk in all service areas to determine effectiveness of implementation, quality of service and training needs.  This position will ensure services are delivered in compliance to applicable safety standards and agency policies and procedures and practices.

This position will review client profiles as assigned, as the ‘qualified person’ per Creating Excellence Together (CET) Accreditation Standards, in situations where a Support Approach consultant is not assigned. Within this role, the CS Coordinator will make revisions as required and follow Agency processes for profile development and review.

The CS Coordinator will facilitate client safety and address risk through the promotion of skill development  for all clients in service.  This will be demonstrated through the development of client focused training by support staff and would be included within the client’s profile.

As part of agency strategic planning, this position will be responsible for updating the Performance and Quality Improvement (PQI) document annually. This will involve a summary of the agencies goals to be submitted to the Agency Planning Committee.

The CS Coordinator will complete related administration task for the Support Approach Team.

Qualifications & Requirements

  • Degree or diploma in human services /occupational health or related field
  • 3 years supervisory experience required
  • 5 years experience working with individuals with a developmental disability.  Experience with complex needs preferred
  • Excellent verbal, written, presentation and interpersonal skills
  • Safety and behavioral program experience related to individuals with complex health needs

Typical Schedule

This position will work a flexible 40-hour week which may include evenings, weekends, and early mornings (e.g. home support hours).

Compensation

Starting annual salary of $ 64,801.68 and we offer a competitive benefits package.
